Do You use mods or changed something in files?
Check also “level4” in images. (thought I dont think that it is something wrong with that).

Looks like something’s wrong with your hard drive… maybe it’s too cold. Where do you live? I’ve heard this can be fixed by heating up the data in the microwave oven then putting it back in your computer? I’m not so sure if that works though.

Try update the drivers of your graphics card.
